Ever wondered how blood still returns to the heart even when major chest veins are blocked? 🫁🩸 It’s because of a hidden backup system called the azygos system. The azygos venous system is a network of veins in the thorax that drains deoxygenated blood from the posterior chest wall into the superior vena cava. Think of it as the body’s emergency drainage highway. It is made up of three main veins: azygos vein, hemiazygos vein, and accessory hemiazygos vein. The azygos vein is the main one, located on the right side of the vertebral column. It is usually formed from the union of the right subcostal vein and ascending lumbar veins. It ascends through the posterior mediastinum and drains into the superior vena cava. What makes it important? It collects blood from most of the right posterior intercostal veins and also receives blood from the left-sided veins through cross connections. The hemiazygos vein is found on the left lower thorax. It drains the lower left posterior intercostal veins. It ascends and then crosses over to join the azygos vein around T8–T9. This crossing is key — it allows left-sided blood to reach the right-sided system. The accessory hemiazygos vein sits higher on the left side. It drains the middle posterior intercostal veins and also crosses to join the azygos vein or sometimes the hemiazygos vein. Basically, it completes the left-side drainage pathway. Clinically, the azygos system becomes very important when the inferior or superior vena cava is blocked. Instead of collapse, the body reroutes blood through this system. It can also become enlarged in conditions like SVC obstruction or portal hypertension and is visible on imaging studies. In simple terms: this is the body’s hidden backup plan for thoracic blood drainage.
